Member Freeze Request Form

Members:
Please complete the following request form to activate your freeze benefit.

• Full Tennis and Health Members may freeze any four months out of the calendar year.
• Junior Members may freeze any two months out of the calendar year.
• Adult frozen dues are half of the normal monthly rate.
• Children may freeze an unlimited number of months per year.
• Child frozen dues are zero.
• Freeze requests must be submitted by the 25th of each month, prior to the month you wish to freeze.
• You may freeze from the first through the end of the month or mid-month from the 15th to the 15th.
• Please note: Corporate, 30, or 60 Day Memberships are not eligible for the freeze benefit
